Cedars-Sinai Registered Nurse - Lead Heart & Lung Transplant Coordinator - 12 Hour Days in California

Job Description

Cedars-Sinai is among the few medical centers in Southern California with Medicare certification for lung transplants. The center blends the multidisciplinary of an experienced team of surgeons, pulmonologists, diagnostic imaging specialists and specially trained transplant nurses to provide groundbreaking care for patients who can benefit from lung transplantation.

The Lead Transplant Coordinator is responsible for the planning and coordination of all aspects of care for patients throughout the continuum of transplant services. They will oversee donor offers and coordinate with all interdisciplinary services to determine viability. Will provide assistance to Transplant Coordinators for complex cases.

As an experienced member of the multidisciplinary team, their role will lead efforts to supervise and improve referral, evaluation and waitlist workflow as well as patient outcomes and performance improvement projects in addition to participating in peer onboarding, training and mentorship.

Lung transplant patients at Cedars-Sinai have access to all of the services and programs for which the Medical Center is renowned. These include the S. Mark Taper Imaging Center, with groundbreaking diagnostic technologies; innovative surgical suites; and the latest in new techniques such as video-assisted thoracoscopic surgery, lung volume reduction surgery and minimally invasive surgical techniques and interventional pulmonology.

Essential Job Duties:

  • Serves as the lead coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dietitians, pharmacists, financial analysts, case managers, support staff and other transplant coordinators facilitating communication and evidence based practice.

  • Expert in planning and coordination all aspects of care for patients throughout the continuum of transplant services.

  • Participates in staff evaluation including probationary period, annual appraisal and counseling for nursing and administrative staff within respective transplant phase.

  • Participates in employee engagement efforts.

  • Develops weekly and monthly staffing and on-call schedules, approves vacation requests and leads daily/weekly staff meetings and huddles

  • Monitors staffing ratios in respective transplant phase and implements strategies to improve workflow.

  • Collects/documents data to meet clinical, regulatory (OPTN/UNOS/MCS), financial requirements.

  • Leads the development and implementation of practice guidelines, protocols, pathways, performance improvement and research activities.

  • Coordinates policy development and required updates with Quality Manager related to UNOS required guidelines and revisions to allocation and regulations.

  • Undertakes a leadership role in improving the quality of transplant nursing care and serves as a consultant to the interprofessional team. Acts as nurse expert to represent transplant program during audits and regulatory surveys.

  • Provides nursing care consistent with Nursing Standards of Care/Practice, the Plan for Provision of Nursing Care, The Plan for Provision of Care to Patients, CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan and Nursing Strategic Plan.

  • Identify safety concerns and institute measures to provide a safe patient care; and provide appropriate assessment, intervention and evaluation of patients with consideration of clinical and developmental milestones in the patient population.

  • Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community. Represents the Medical Center at regional and national meetings/conferences.

  • Maintains competency skills including the ability to provide care to special patient populations and patient with diverse cultural backgrounds.

  • Provides and monitors professional development such as mentorship and educational opportunities to those on the interdisciplinary team.

Qualifications

Education:

  • BSN degree required or Masters preferred

License/Certifications:

  • Current California RN License required

  • BLS from the American Heart Association required

Experience:

  • A minimum of 5 years transplant coordinator experience required

  • Certified CCTC (ABTC) required
