What you will be doing in this role?

The Clinical Social Worker provides a full range of psychosocial services, in collaboration with multidisciplinary staff members, to meet the clinical and discharge planning needs of patients at Cedars-Sinai. The Social Worker will have opportunity to participate in research, educational programs, and administration of clinical services. They will work in close collaboration with psychiatry.

Focused on providing supportive services to medical patients with substance use disorders. They will be expected to use motivational interviewing to assess and improve readiness for treatment; Help patients plan for linking to substance use disorder treatment after discharge; Facilitate linkage to treatment by helping the patient make appointments and reduce barriers; Conduct four follow-up calls with each patient after discharge; Collaborate with the primary medical team on treatment and discharge planning; Assess for risk factors and guide patient through safety planning and crisis management if needed

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

  • Psychosocial evaluation -- identification of problems affecting optimal patient care and the development of a treatment plan

  • Develops and maintains productive relationships with community health, welfare, and social agencies

  • Interdisciplinary discussion of psychosocial implications of illness including consultation regarding treatments and resources designed to solve the problems of the patients

  • Participates in development and leadership with Department's Continuing Education

  • Complies with regulatory requirements (i.e. BBS) appropriate for position.

  • Respond in a timely manner to community inquires and referrals initiated by various members of the medical teams

  • Provide brief behavioral interventions

  • Discharge planning and case coordination

  • Advocate for patient rights and self-determination in the medical system

  • Complete comprehensive documentation in the patient’s electronic medical record, consistent with legal and organizational requirements

  • Collaborate with the transdisciplinary treatment team of physicians, nurses, social workers, psychologists and medical staff to ensure integration of care

Educational Requirements:

Graduate of an accredited School of Social Work with a Master’s Degree.

Licenses:

California License

Experience:

Operates as the primary social worker for an assigned service or program. Knowledge and experience taking care of patients with mental health and substance use disorders. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served within the entire Department with a specific focus for adults (over the age of eighteen (18)). Demonstrates knowledge of the principles of growth and development over the life span, with and emphasis on adults, and possesses the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age-specific needs, and to provide the care needed as described in the unit’s/area’s/Department’s policies and procedures. Despite emphasis with adults, has an understanding of developmental milestones throughout the age continuum with the ability to obtain and interpret information specific to and on behalf of the Department’s overall patient population. Through direct observation, case discussion, and documentation review, the Licensed Clinical Social Worker demonstrates this ability.
