What You be doing in thisRole:

Provide a

full range of social work services in collaboration with other CSMC staff to

meet the psycho-social assessments, crisis intervention, Care Coordination,

transitions management and discharge planning needs of patients and families.

Preoperative, Perioperative and Postoperative evaluation and treatment of

psychosocial issues, in the transplant patient population.

What We are Looking for:

  • Familiaritywith transplantation.

  • Demonstrated ability to work with a multidisciplinaryteam

Requirements:

Education: Master of Social Work (MSW) required.

License : Licensed Clinical Social Worker (LCSW) required.

Experience:

  • A minimum of 2 years' Acute Care Hospital experience required.

  • Experience working with transplant patients preferred.
