What will I be doing in this role?

A Psychiatric Social Worker (PSW) collaborates with an

interdisciplinary team in providing care to the psychiatric patient and is a

resource for all caregivers, the patient and the patient’s family. In

conjunction with the licensed independent provider, provides education and

information on available options, the informed decision making process, and

realistic goal setting in order to empower the patient/caregiver to actively

participate in their plan of care. Establishes and coordinates a supportive

care plan with goals and interventions based upon treatment and individual

patient/caregiver needs, preferences, beliefs and values. Keeps

patient/caregiver involved in plan and moving toward self-directed care.

Reviews, evaluates and revises the plan of care for appropriateness on an

ongoing and timely basis. The PSW is an advocate for the patient/caregiver. The

PSW assesses the ability of the patients to follow instructions and give

consent. He/she identifies safety concerns and institutes safety measures.

Facilitates multidisciplinary conferences when indicated by complexity of patient’s

psychiatric condition or other barriers to care:

  • Assists in the assessmentof “holdable” status and in decision making related to restraint use.Writes and removes 5150 hold.

  • Coordinate all transfersto psychiatric facilities, both voluntary and involuntary, includingattainment of insurance authorization, where applicable.

  • Complete Columbia SuicideSeverity Rating Scale (CSSRS) as indicated and provides appropriatereferrals

  • Develop collaborativeplans for high-utilizers with psychiatric comorbidities.

  • Proficiency navigatescommunity systems of care. Provides appropriate resources and referralsbased on individual need.

  • Provides Continuing YourCare (CYC) referrals to Saban Clinic.

  • Observes, evaluates andexpeditiously alters plan of care based on signs and symptoms ofpsychiatric distress.

  • Identifies psychiatriclevel of care for appropriate, safe, and complete transition of care.Plans and implements individualized patient care in collaboration withinterdisciplinary teams based on observations. Implements appropriatereporting, referrals, and care in accordance with standardized procedures.Initiates emergency procedures when indicated. Assists in dischargeplanning and act as a liaison between ED team, PET team, and TransferCenter.

  • Provides care to specialpatient populations and patients with diverse cultural backgrounds.Provides crisis support and therapeutic counseling

  • Identifies and makespecial adjustments as required to the specific populations needs,including cultural, spiritual, age, psychosocial, communication, gender,sexual orientation, economic, education, family and condition needs.Provides education for patient to navigate community support.

  • Identifies and assessespatient safety concerns with respect to age and developmental considerations.Intervenes to provide a safe environment and evaluates effectiveness ofintervention for patient.

  • Provides assistance toco-workers, new staff, temporary staff and students. A resource to stafffor psychiatric related clinical questions/concerns.

  • Works collaboratively withall ED staff, Social Workers, Case Managers, Community ResourceCoordinators (CRC’s). Consults with Psychiatry

  • Consults with DMH forreferrals to outpatient clinics.

Requirements:

  • Masters Degree in Social Work required.

  • Valid Clinician Social Work license, Psychiatric Mental Health Certification and any other competencies required by your unit within the first year.

  • Two or more years of hospital social work experience or healthcare related experience, preferably in Psychiatric or ED setting preferred.

Skills Sought:

  • Experience in psychosocial assessments, interventions, and crisis counseling.

  • Excellent interpersonal communication and conflict resolution skills in interactions with patients, families, physicians, and the healthcare team.

  • Self-directed and tolerates frequent interruptions and a demanding workload.

  • Experience providing quality patient care and customer satisfaction, in a safety and cost efficient manner with a commitment to continuous quality improvement.
