As a Registered Nurse, Care Coordinator at Cedars-Sinai,

  • You will coordinate the care and service of the patient population from admission through discharge.

  • You will assess and Plan, implement, monitor, and evaluate options and services to meet an individuals health needs to facilitate a safe and timely discharge to the next appropriate level of care.

Educational

Requirements:

  • BSN required

License/Certification/Registration

Requirements:

  • Current CA RN license

  • American Heart Association, BLS

  • Case Management Certification required within 2 yearsof hire.

Experience:

  • 3-4 years Case Management experience preferred

  • 2 years related nursing experience required.

  • Knowledge of medical literature, research methodology,health care delivery systems and financial reimbursement.
