As a Registered Nurse, Care Coordinator at Cedars-Sinai,

You will coordinate the care and service of the patient population from admission through discharge.
You will assess and Plan, implement, monitor, and evaluate options and services to meet an individuals health needs to facilitate a safe and timely discharge to the next appropriate level of care.
- BSN required
Current CA RN license
American Heart Association, BLS
Case Management Certification required within 2 yearsof hire.
3-4 years Case Management experience preferred
2 years related nursing experience required.
Knowledge of medical literature, research methodology,health care delivery systems and financial reimbursement.
