As a Registered Nurse,
Care Coordinator at Cedars-Sinai,
You will serve as a clinical co-pilot—charting the besttrajectory of each patient’s care by accommodating various needs,including medical, psycho-social, financial and practical.

You will coordinate the care and service of the patientpopulation from admission through discharge.
You will assess and Plan, implement, monitor, andevaluate options and services to meet an individuals health needs tofacilitate a safe and timely discharge to the next appropriate level ofcare.

- BSN required
Current CA RN license
American Heart Association, BLS
Case Management Certification required within 2 years of hire.
3-4 years Case Management experience preferred
2 years related nursing experience required.
Knowledge of medical literature, research methodology, health care delivery systems and financial reimbursement.
